This form enables you to accept all the responsibilities of taking over as the booking parent (client-parent) for a child accessing service at our practice. Please read the form carefully as you progress so that you understand your obligations.
What to have on hand to complete this form:
- Your NDIS number and Plan Manager details (if relevant).
- Your Medicare card details (if relevant).
- Contact details for any approved contacts (eg. other family members or teachers, etc).
